San Francisco agreed to terms on a deal with the All-Star, according to ESPN's Buster Olney.
The deal is pending a physical.
Melancon, who will turn 32 years old in March, went 2-2 with a 1.64 ERA last season.
He racked up 47 saves and registered a 0.897 WHIP.
