NTSB releases report on deadly pipeline explosion

Federal accident investigators have released a 140-page report on a gas pipeline explosion that killed eight people and decimated a suburban neighborhood near San Francisco last year.

The National Transportation Safety Board report posted online Monday pulls together information gathered in the board's yearlong investigation of the Sept. 9, 2010 explosion and fire in San Bruno.

It includes an analysis of the causes of the accident and a series of recommendations to gas pipeline operators and state and federal regulators.       

The board unanimously agreed at a meeting last month that the accident was caused by "a litany of failures'' by the Pacific Gas & Electric Co., one of the nation's largest gas companies, as well as weak oversight by regulators.       

The board also posted a video summarizing its investigation.
