This link takes you to the California Dept. of Public Health search tool.You can look up individual hospitals and long term care facilities to see if they’ve been fined for immediate jeopardy or adverse event violations. SHC denied any liability in this matter and vigorously defended the lawsuit. The civil lawsuit has been resolved and we are not at liberty to discuss the terms of its resolution. Matters are often resolved due to the extensive costs of litigating disputes; such resolutions are not intended to be any acknowledgment of fault. The California Department of Public Health did a review and made suggestions for improvement, which were acknowledged and responded to; however, there was no fine or penalty assessed as a result of that review. Nurses and physicians often keep temporary notes that are never intended to become part of the medical record. Once the formal entry is made into the permanent record, the temporary notes are then discarded.”
