Matters are often resolved due to the extensive costs of litigating disputes; such resolutions are not intended to be any acknowledgment of fault. The California Department of Public Health did a review and made suggestions for improvement, which were acknowledged and responded to; however, there was no fine or penalty assessed as a result of that review. Nurses and physicians often keep temporary notes that are never intended to become part of the medical record. Once the formal entry is made into the permanent record, the temporary notes are then discarded.”
