Equipment Malfunction Caused Massive SF Blackout

We’re learning more about what caused a massive blackout in San Francisco in April. A company hired by PG&E to investigate the incident concluded that the problems involved equipment failure at an electrical substation and a flawed design of a critical alarm system.
